Please call the office if you do not see your insurance company listed here. The patient is responsible for paying all deductibles and co-pays at the time of service. If you are unsure of your deductible or co-pay, please call your insurance company before your appointment to find out how much your co-pay and deductible is and how much of the deductible you have met.
